WESTBOROUGH, Mass.—September 15, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that New Jersey Physicians ACO has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for population health management to advance its Accountable Care Organization (ACO)-related objectives. This relationship will aid the physician-led organization in administering the ACO in a more efficient manner, including capturing quality metrics and performing analytics.
“New Jersey Physicians ACO formed to provide a platform that rewards independent practices for delivering quality care. We have a huge emphasis on ensuring the Primary Care Physician becomes the central player in coordinating the overall care of the patient,” said Dr. Vinod Sancheti, founding member of New Jersey Physicians ACO and a Board Certified Internist practicing in New Jersey. “We turned to eClinicalWorks because of its strong functionality and success with primary care-focused ACOs, such as ourselves. Working with eClinicalWorks will provide a deep level of insight into our patient populations and care coordination. Our aim is to partner up with more primary care offices that believe in the independent ACO model and, as we do that, eClinicalWorks will be able to help us scale.”
The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship. Recently recognized by Frost & Sullivan with its 2014 North America Frost & Sullivan Award for Enabling Technology Leadership, eClinicalWorks CCMR will work closely with New Jersey Physicians ACO in meeting its quality measures and performance metrics.
“Physician-led ACOs have had success with lowering costs while improving quality,” said Girish Navani, CEO and co-founder of eClinicalWorks. “The eClinicalWorks CCMR platform has been recognized for including all the major technology areas needed to successfully implement a population health management initiative, such as powerful analytics and a vision into patient populations that is actionable at the point of care. We look forward to partnering with New Jersey Physicians ACO.”
eClinicalWorks CCMR will be providing New Jersey Physicians ACO with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
